how to save template file as different name?
usually youhave normal.dot as your template file
but I want to have my own template file when loading
custom addin

Use File>Save As, select Word Template in the Files of Type drop down,
select the Word\Startup Folder as the destination and give it a name.

Sorry my question was not clear.
at the first time, when I launch my word
create my template file named myaddin.dot and save

Application.Templates("C:\myaddin.dot").Save
